The Quality Excellence Coordinator will serve as a leader, collaborating with clinic teams to create an environment of high quality care and patient experience. The Quality Excellence Coordinator understands and operationalizes the PQRS, eCQM, and VBM Measures and facilitates performance improvement teams to achieve compliance with each measure set. Provides support in the administration of SRHC's performance improvement plan and facilitates and provides data to performance improvement teams. Develops, coordinates, and implements systems for the identification, collection, reporting, and analysis of performance measurement data. The Quality Excellence Coordinator facilitates the patient experience strategic objectives at SRHC and provides data to patient experience teams. The position functions as an objective role that reviews and evaluates quality and satisfaction issues/concerns within the health center. The Quality Excellence Coordinator has the authority to review all documents and other information relative to quality and satisfaction activities, including but not limited to, patient records. The Clinic Quality Excellence Coordinator exists as a channel of communication to receive and direct quality and satisfaction issues to appropriate resources for investigation and resolution.
Associate's degree in a healthcare related field
Bachelor's degree in nursing preferred
Lean Six Sigma Black/Green Belt training preferred
Two years experience in quality in a healthcare environment
