Infection Control and Chronic Care Coordinator (Full Time)
The Infection Control and Chronic Care Coordinator is a qualified individual with knowledge of CDC guidelines and definitions of healthcare associated infections (HAIs), Joint Commission standards, federal and state regulations. She/He is responsible for developing and implementing in-service programs, policies and procedures related to infection prevention and control. Is responsible to ensure success of the CCM program. The CCM Coordinator is responsible to develop, monitor, analyze and report key performance metrics, develop and maintain patient registries, develop and operationalize procedures for the success of the chronic care management program.
