Under the direct supervision of the Manager of Quality Management, the Clinical Quality Management Coordinator is responsible for Patient Centered Medical Home Recognition for all Springfield Clinic Primary Care practice locations. To achieve and maintain recognition, the QM Coordinator works with the clinical teams in Primary Care locations to develop processes designed to meet and maintain the standards required for NCQA Patient Centered Medical Home Recognition status.
