Plan and direct medical policies and programs. Define and achieve financial targets in support of business goals; evaluate organizational functions and structure to best determine the allocation and utilization of services.
Facilitate strategic clinical relationships with physicians. Work with leadership to develop a strategic plan for growth of clinical practice.
Ensure that clinical programs are in compliance with governmental and regulatory requirements.
Implement utilization review and quality improvement systems. Implement educational programs and offerings.
May provide medical services to patients in an assigned area.
Prepare annual operating budget for assigned service areas.
Consult with planning and operations leadership in new program development. Supervise any assigned medical/resident programs.
The position reports directly to the CMO of AMG WI and will partner with their operations dyad partner.

5-10 years of progressive leadership experience in population health management as well as quality.
Excellent physician relations skills and proven experience growing service line volume in a multi-facility health system.
Skills in relationship development and collaboration across a matrix environment to establish a shared vision and strategy for quality and population health
Proven effective and innovative business development expertise, experience, and skills gained within an organization known for leading-edge program development in an organization committed to care excellence.

knowledge of population health operations in managed care, including quality management, outcomes, provider network, claims, financial management, care management policies, disease state and population management, members’ rights and responsibilities, and the ability to quickly understand and assess program compliance.
The ideal candidate will be a clinical leader and content expert in quality measures, population health operations, case and population management, and experience working with a clinically integrated network and ACO. The candidate must exhibit strong team leadership, project management and organizational skills, as well as credibility with clinicians and medical group leadership. Is able to create and execute the implementation plan to deliver a strong care management model and engage network providers in positive trends and improving overall population quality and cost outcomes. Will provide strategic clinical oversight of all value based agreements in the Wisconsin market.
Works with key members of the population health management team on the ongoing development and implementation of operational processes, strategic initiatives, policies and procedures used in delivery of care management services, as well as the ongoing integrated population analytics to manage and drive care management and other PHM workflows.
Focus on the ongoing management and outcomes driven performance improvement of the population health initiatives/programs in conjunction with the operations dyad director and other clinical teams. The dyad team is responsible for developing and implementing multi-disciplinary performance improvement teams that focus on identifying root causes to lagging performance metrics and then identifying, prioritizing, and implementing solutions aimed at improving program outcomes. Focus on resolving staff issues, completes performance reviews, assists in the development, implementation and monitoring of quality and compliance activities, assists in the reviewing and revising department standards and policies and coordinates the orientation of new employees to the department. Will continuously work at identifying gaps in staff education and work with staff to close gaps.

Important Information for CPHQ Recertification
The recertification process has been streamlined. Unlike previous years, you don’t need to list out your CE activities at the time of completing your application. Instead, you will be asked to attest to having met the requirements, further aligning with the honor-based system of recertification. In the event that your application is randomly selected for audit, you may be required to list your CE activities and submit supporting documentation.
While you may participate in NAHQ learning opportunities (Learning Labs, JHQ articles, etc.), you can only claim CE credit once for each activity. Therefore, if you’ve previously earned CE from a NAHQ learning opportunity, you can’t claim it again, nor will you see the repeat CE appear in your “My Learning� tab. This includes activities used in previous recertification cycles.
Remember, if you’re recertifying during the grace period (1/1/24-1/31/24), you already had to earn your CE hours by Dec 31, 2023.
