Description
Lutheran Social Services of WI & UP MI seeks a Long-Term Care Consultant to strengthen quality and compliance across our long-term care and residential programs. In this mission-driven, faith-inspired nonprofit, you will assess services, review documentation, and ensure adherence to state and federal regulations while promoting trauma-informed, person-centered care. You’ll coach and train staff, lead quality improvement initiatives, and collaborate with leaders to enhance client outcomes. This hybrid role offers supportive leadership, ongoing training, and opportunities to innovate in care delivery and impact vulnerable individuals and families.Provide consultative support to long-term care programs and residential services across LSS locations in WI and Upper MI
Assess care quality, regulatory compliance, and risk in long-term care settings; recommend improvements and action plans
Train and coach staff on best practices in person-centered, trauma-informed, and culturally responsive care
Collaborate with program leaders to design, implement, and evaluate quality improvement initiatives
Review documentation, care plans, and incident reports to ensure accuracy and adherence to standards
Monitor state and federal long-term care regulations and communicate changes to stakeholders
Prepare reports, metrics, and summaries for leadership highlighting trends and performance
Partner with interdisciplinary teams to address complex client needs and care transitions
Support accreditation, licensing reviews, and internal audits
Participate in strategic planning to enhance long-term care service delivery and client outcomes
Long-term care regulatory knowledge (state and federal)
Healthcare quality improvement methodologies
Clinical assessment and care planning
Audit and compliance review
Data analysis and reporting for quality metrics
Policy and procedure development
Staff training and facilitation
Risk management in care settings
Electronic health record (EHR) documentation
Knowledge of trauma-informed and person-centered care practices

Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) license preferred
Certified Dementia Practitioner (CDP) or similar dementia care certification a plus

Knowledge of CMS and state long-term care regulations
Training in trauma-informed care or person-centered care models preferred
