Description

As Manager of Case Management at Memorial Hermann Health System, you will lead a team of case managers to coordinate safe, efficient, patient-centered care across the continuum. You will oversee utilization management, discharge planning, and transitions of care, ensuring regulatory compliance and alignment with quality and cost goals. In partnership with physicians, nursing, and interdisciplinary teams, you’ll develop and implement best practices, analyze performance metrics, and drive process improvement. Lead and supervise case management staff, including hiring, coaching, and performance evaluations.
Oversee utilization review, discharge planning, and care coordination processes.
Ensure compliance with regulatory, payer, and accreditation requirements.
Collaborate with physicians, nursing, and interdisciplinary teams to optimize patient flow and outcomes.
Develop, implement, and monitor case management policies, procedures, and best practices.
Analyze case management metrics, identify trends, and lead quality and process improvement initiatives.
Manage departmental budgets, staffing levels, and resource allocation.
Support complex patient cases, escalations, and barriers to safe, timely discharge.
Educate staff and stakeholders on case management standards and care coordination strategies.
