Description
Liberty Health seeks a detail‑oriented Quality Assurance Coordinator to support our Healthcare & Medical Services team. In this role you will monitor clinical and administrative processes, conduct chart and documentation audits, track quality metrics, and ensure compliance with healthcare regulations and internal policies. You will collaborate with case managers and interdisciplinary teams to identify gaps in care coordination, implement corrective action plans, and support staff education. Working in a patient‑centered, inclusive culture, you’ll help drive continuous improvement that enhances care quality, safety, and outcomes across our continuum of services.Conduct regular audits of patient charts, documentation, and workflows for accuracy and completeness
Monitor quality indicators, patient safety metrics, and performance dashboards
Support development and implementation of quality improvement initiatives and corrective action plans
Ensure compliance with regulatory, accreditation, and internal policy standards
Collaborate with case managers and clinical teams to improve care coordination and outcomes
Prepare quality reports and summaries for leadership and committees
Assist with incident reporting, tracking, and follow‑up activities
Provide staff education and feedback on quality standards and documentation best practices
Standardize and maintain QA procedures, checklists, and process documentation
Use data and analytics tools to identify trends, risks, and opportunities for improvement
Clinical documentation review
Healthcare quality improvement (QI) methodologies
Healthcare regulations and compliance (e.g., CMS, Joint Commission)
Data analysis and reporting
Electronic Health Records (EHR) systems
Care coordination workflows
Audit and root cause analysis
Process mapping and standardization
Incident reporting and risk management
MS Excel and quality dashboards
Medical Insurance
Dental Insurance

Licensed RN or other clinical license preferred
Certified Professional in Healthcare Quality (CPHQ) preferred
Experience with Joint Commission or similar accreditation standards
Training in quality improvement or Lean/Six Sigma (e.g., Yellow/Green Belt) preferred
