Description
VNS Health is seeking a Quality Improvement Specialist to advance excellence in home and community-based care. Working within the Quality Assurance & Testing department, you will analyze performance data, identify improvement opportunities, and support teams in implementing evidence-based quality initiatives. Responsibilities include developing and monitoring quality metrics, conducting audits and root cause analyses, and ensuring alignment with regulatory standards. Collaborating closely with clinical and operational leaders, you will help design better workflows, enhance patient safety, and promote patient-centered outcomes. Support design and implementation of quality improvement initiatives using evidence-based methodologies.
Develop, track, and report key performance indicators and dashboards for leadership and frontline teams.
Conduct audits, chart reviews, and root cause analyses to ensure compliance with regulatory and accreditation standards.
Collaborate with clinical, operational, and administrative teams to implement corrective action plans.
Facilitate quality-focused meetings, workgroups, and training sessions to build quality culture.
Prepare clear reports and presentations that communicate findings and recommendations.
Partner with staff to optimize workflows and patient-centered care processes.
Monitor sustainability of improvements and adjust interventions as needed.
Contribute to a culture of compassion, integrity, and collaboration aligned with VNS Health values.
Quality improvement methodologies (PDSA, Lean, Six Sigma)

Root cause analysis
Regulatory and accreditation standards (e.g., Joint Commission)
Process mapping and workflow redesign
Electronic health record (EHR) reporting
Performance dashboard development
